Short answer

White spots after braces are areas of demineralisation that form when plaque collecting around the brackets draws minerals out of the enamel. Some superficial spots may fade in the first months with saliva and fluoride. For permanent spots, resin infiltration, commonly known as ICON treatment, is often used. Whitening does not treat the spots and can make them look more noticeable at first.

Why do white spots form after braces?

White spots after braces form when acids produced by plaque that stays around the brackets for a long time draw minerals out of the enamel surface. This is called demineralisation, and a white spot is considered the earliest stage of decay.

Brackets and wires create small recesses that are hard for a toothbrush to reach. Plaque builds up more easily in these areas. The following increase the risk:

  • Not cleaning around the brackets thoroughly enough
  • Frequent sugary snacks and sugary drinks
  • Sipping acidic drinks throughout the day
  • Not using fluoride toothpaste regularly
  • Missing check-ups and professional cleaning

The spots usually appear as a crescent or frame shape matching the edge of the bracket and are often noticed when the braces come off.

What is the difference between a white spot and decay?

A white spot is early enamel loss where the surface has not yet broken down; decay is when this process has progressed and formed a cavity. White spots caught early can often be stopped.

FeatureWhite spotAdvanced decay
AppearanceMatt, chalky white areaBrown or dark, with a cavity
SurfaceUsually smoothRough, broken or pitted
SensitivityUsually noneMay occur with cold, heat or sweet things
ApproachRemineralisation, resin infiltrationA filling or another restoration

If the spot is getting darker or its surface is starting to feel rough, do not delay an examination.

Do white spots go away after braces?

Some superficial white spots may fade somewhat on their own in the first months after braces come off; however, most do not disappear completely. They fade because plaque build-up decreases and minerals in saliva return to the enamel surface.

That is why spots are usually not treated straight away but monitored for a while first. The monitoring period is decided by the dentist based on the depth and appearance of the spot. Taking photos during this time helps track the change objectively.

What can be done in the first months?

In the first period after braces come off, the aim is to restore minerals to the enamel and prevent new spots from forming. This is called remineralisation.

  1. Brush twice a day with fluoride toothpaste; after brushing, do not rinse your mouth with lots of water, just spit out the toothpaste.
  2. Use a fluoride mouthwash if your dentist recommends it.
  3. Fluoride varnish can be applied at the clinic.
  4. Supporting products containing calcium and phosphate recommended by the dentist can be used.
  5. Limit sugary and acidic drinks between meals.
  6. Clean your retainer and your teeth every day.

If there is tartar and plaque build-up, a scale and polish is done first; a clean surface also makes remineralisation easier.

Which treatments are used for permanent spots?

For spots that are still noticeable after the monitoring period, there are several options. The choice depends on the depth and number of spots and on what you expect.

  • Resin infiltration (ICON treatment): The pores of the spot are filled with a flowable resin; no tooth tissue is drilled away. It is often used for spots after braces.
  • Microabrasion: For very superficial spots, a very thin layer is removed from the enamel surface.
  • Cosmetic filling: May be preferred for spots that have formed a cavity or are deep.
  • Veneers: Considered for widespread, deep spots when other options are not enough.

We explain how resin infiltration is carried out in detail on our page white spot treatment.

How does whitening affect white spots?

Whitening does not treat white spots. Because the teeth temporarily lose water during treatment and in the first days afterwards, the spots can look more noticeable.

As the teeth regain moisture, this effect lessens. In some people, once the overall tooth colour is lighter, the difference between the spot and its surroundings may stand out less; however, this varies from person to person and cannot be predicted in advance.

Planning

If both whitening and spot treatment are being considered, whitening is usually done first. Once the colour has settled, usually after waiting 1–2 weeks, spot treatment follows. This way the resin colour matches the new tooth shade.

For the timing of whitening after braces, see our page teeth whitening after braces.

How can white spots be prevented during braces treatment?

The key to preventing white spots is keeping the area around the brackets free of plaque throughout treatment. As orthodontic treatment is also provided at our clinic, we explain care habits to patients getting braces at the start of treatment.

  • After every meal, or at least twice a day, brush above and below the brackets separately.
  • Use an interdental brush and a floss threader.
  • Use fluoride toothpaste regularly.
  • Limit sugary and sticky foods.
  • If plaque build-up is noticed at check-ups, have a professional clean.

These measures noticeably reduce the risk of spots you may find when the braces come off.

When should you see a dentist?

It is advisable to have any white spot you notice after braces come off looked at once at an examination. Also, do not delay a check-up if:

  • The spot is turning yellow or brown
  • The spot is growing or its surface is becoming rough
  • Sensitivity to cold, heat or sweet things has started in that area
  • The spot has not changed at all after a few months and it bothers you

Whether the spot really is demineralisation or is due to another cause such as fluorosis is determined at the examination. For other types of stain, take a look at our page tooth yellowing and stains.

Do treated spots come back?

Spots usually do not come back in the treated area; however, if plaque build-up continues, new spots can form on neighbouring surfaces. Keeping the result therefore depends largely on daily care.

  • Keep brushing regularly with fluoride toothpaste.
  • If you have a fixed retainer, clean around it carefully.
  • Do not skip your check-up appointments.

Surfaces treated with resin infiltration can also pick up colour over time, just like natural teeth. Regular polishing and cleaning help keep their appearance for longer. At our clinic in Oran, Çankaya, we can monitor your spots together with your check-ups after braces.

Frequently asked questions

Do white spots after braces go away on their own?

Some superficial spots may fade somewhat in the first months, but most do not disappear completely. For permanent spots, treatment options are assessed at an examination.

How long should I wait with white spots?

There is no fixed period. Spots are usually monitored for a while; your dentist decides when to treat them based on their depth and how they change.

Does ICON treatment work on white spots from braces?

Resin infiltration, known as ICON treatment, is often used for superficial and medium-depth spots after braces. Results may be limited for deep spots.

Does whitening cover up white spots?

No, whitening does not treat the spots and can make them look more noticeable in the first days. In some people the colour difference may stand out less over time.

Is a white spot decay?

A white spot is considered the earliest stage of decay. At this stage the surface has not yet broken down and, with good care, its progression can often be stopped.

Does white spot treatment after braces hurt?

Because no tooth tissue is drilled away in resin infiltration, there is usually no pain and in most cases no numbing is needed. For options such as fillings, numbing is used if needed.
